PAVE-CKD Patient navigator program for patients with early chronic kidney disease

Have you ever felt you needed more information and support to help manage chronic kidney disease? You are invited to the PAVE-CKD workshop to discuss the needs of patients in managing chronic kidney disease. Your opinion and suggestions will inform a study (clinical trial) of a patient navigator program to help people with early chronic kidney disease understand and manage their health, and to access healthcare. The workshop will involve about 30 patients with chronic kidney disease and family members.

When: 10:00 am – 1:00 pm Saturday 11th May 2019

Where: Mercure Sydney hotel, 818-820 George St, Chippendale, NSW 2007 (next to Central Station)

Cost/reimbursement: The cost if free. Patients/caregivers will be reimbursed $30. Morning tea and lunch will be provided.
